HEALTHIER WY HEALTH ASSESSMENT

In order to qualify for the 2011 $480 Insurance Premium Discount, members need to complete the Health Assessment at the Great-West website by September 30, 2010. Follow these following steps:
- Visit the Great-West website (http://www.mygreatwest.com)
- Click on the "Register Now" section
- Type in the group number 00052665
- Enter your member ID that is on your Great-West card
- Follow the rest of the instructions and hit submit.
You will then be taken to the next page where you pick your our user name and password. After you create your user name and password:
- Navigate to the welcome page, then to the Wellness Tab, and then Health and Wellness
- Read the disclaimer and hit submit.
On the right side of the page, there should be a Health Assessment box. Click on it and that will take you to the assessment. If you have any questions regarding user name, password or any other website issues, please call:

After taking the Health Assessment, make sure you received a score. This will indicate that you have successfully completed the Health Assessment. Your Healthier WY status will be updated in about four weeks and your $50 incentive check will be mailed within six weeks after completing the Health Assessment.
